Best place is where nobody else is lurking. If you are calling CQ or calling somebody else your transmit should be in a clear spot -- no other requirement than that.If you are answering the same rule

What is the correct place to put transmit vs receive cursors ? Relationally. Transmit above or below? And what amount of offset? I imagine the software must have split enabled? Ty qsl

You've hit on the problems. As an EE, a ham for 65 years who uses diversity a lot on CW with my K3, and a retired audio professional, I consider it a very bad idea to combine audio from two receivers

The issue was the macOS Security & Privacy Microphone access not being enabled for WSJTX. Not sure if the update changed the setting but with it enabled we're back working as it should.

Apple update last evening. Today I can hear FT8 on radio (Yaesu FT991a) but WSJTX shows nothing on the waterfall. Radio Preferences shows Test Cat and Test PTT are working properly.
Ideas?

Just dial your rig (it must be capable of doing so!).
E.g. FT8 is not frequency dependent, like FM nor SSB is.
